THE INTER-DISCIPLINARY CONCEPTUALIZATION OF PALLIATIVE CARE AS A SOCIAL SUB-INSTITUTION

Abstract

The health care is considered as a social institution since XX century. The main purpose of health care is in amelioration, rehabilitation and promotion of health. This social institution, according to its implementing functions, comprises four sub-institutions. The analysis of trends of institutionalization of the sub-institution «Palliative medical care» is presented in methodological plane of system of social structural functions (AGIL). From the perspective of dysfunctional practices of sub-institution of palliative medical care developing in Russia, a medicalization of palliative care is discussed.

Abstract

The article presents main characteristics of the conception of medicalization, from its theoretical substantiation. The evolution of ideas of Western researchers of the given problem is analyzed from 1970 to 2010. The increasing of number of studies and theoretical reflexing concerning medicalization resulted in development of such parallel conceptions as medicamentalization and biomedicalization.

Abstract

The normative base and publications were analyzed to review present notions «defect of medical care», «medical error». The absence of unified comprehension of the given definitions in the Russian legal practice was established. The established situation has advantages of pretrial and extrajudicial mode of settlement of disputes (mediation) in case of consideration of conflicts related to medical care support as an instrument directed to quick searching of decision mutually acceptable for both sides, including necessity of compensation of damage done.

THE PERCEPTION OF VALUE OF HEALTH AND HEALTHY LIFE-STYLE BY THE FACULTY OF MEDICAL UNIVERSITIES

Abstract

The article presents the results of medical sociological survey with purpose to study concepts and motivation attitudes of the faculty of medical universities in relation to healthy life-style and its components. The survey results demonstrated that in spite of rather wide-spread aspiration for preservation of one's own health (refusal of harmful habits, regular dispensarization, involvement into sport activities) a significant percentage of the faculty of medical university suffers from inadequate level of commitment to principles of healthy life-style. It is noted that implementation of healthy life-style is prevented besides working load and time deficiency, by number of such «anti-motives» as inner unreadiness and lower level of motivation to healthy life-style.

The article considers problem of non-developing pregnancy as an important social occurrence progressively deteriorating level of birth rate despite achievements of modern medicine. The evaluation was implemented concerning medical social portrait of women with non-developing pregnancy. The reliable relationship is established between developed pathology of pregnancy and presence of somatic diseases, number of abortions and miscarriages and also housing conditions, public professional activity, unfavorable conditions of labor, presence of occupational hazards.

THE QUANTITATIVE ANALYSIS OF SIGNIFICANCE OF SOURCES OF MEDICALIZATION IN SOCIOCULTURAL ENVIRONMENT OF MEGALOPOLIS

Abstract

The number of medical information consumed and learned by population of the Russian Federation increased significantly during recent time. The sociological randomized sampled survey was carried out in 2014-2015 in Rostov-on-Don. The sample included 459 adult residents of Rostov-on-Don. The survey established that 65% of respondents never ponder over why they fall ill or they think of their own health only in case of manifestation of certain symptoms. Furthermore, the relationships are analyzed between level of medicalization and status of respondents in gender, educational and self-preserving aspects. Also cumulated morbidity was measured. The parallel application of parametric and non-parametric Student and Pearson criteria permitted to establish absence of a valuable relationship between levels of morbidity and content of medical knowledge obtained within the framework of spontaneous medicalization.

THE QUALITY OF MEDICAL CARE IN MEDICAL ORGANIZATIONS OF MOSCOW PUBLIC HEALTH CARE SYSTEM IN APPRAISALS OF PATIENTS AND PHYSICIANS

Abstract

The article presents the analysis of results of complex medical sociological survey of effectiveness and efficiency of medical care as main criteria of its quality and accessibility. The survey comprised 1,737 patients of various social groups of population of Moscow and 203 physicians of polyclinics. At self-rating of health, 13.2% of respondents determined it as unsatisfactory, 45.1% as satisfactory and 31.3% as good. In all population groups, the most called-for proved to be polyclinic institutions. The high level of satisfaction of patients with quality of emergency medical care was established in all groups. The quality of medical care in polyclinics and hospitals was assessed significantly lower. The most negatively assessed criteria turned out transition to fee-for-service forms of medical support and time limits of waiting for all modes of medical care, except emergency medical care and district therapist. The evaluation by physicians of their own activity and present problems is characterized by their unanimity in need of increasing of salary (100%) and increasing of typical sectoral standards of time of reception of a patient (96.5%), decreasing of intensity of work of medical personal (92.3%) and reduction of some forms of record cards with the purpose of releasing of time and attention of physician for a patient (88.4%). The disrespectful attitude of patients to medical personnel was mentioned by 77.3% of physicians which is a new phenomenon testifying loss of confidence in patient-physician relationship. The quality of implemented work was assessed equally and rather high by both young and experienced physicians. The positive and negative aspects of reorganization of health care were analyzed. Two alternatives of problems were established to focus plans of development of system of Moscow health care to support accessible and qualitative medical care of population.

THE MONITORING OF SOCIAL AND PHYSICAL GENERAL STATE OF THE STUDENTS: EXPERIENCE OF EMPIRICAL STUDY

Abstract

The article presents the project of longitudinal interdisciplinary study on junction of biomedicine and social sciences concerning detection of students' health in process of education. Health is one of the most important capitals that can be applied with more or less success in the process of self-fulfillment of personality. The higher education is a significant factor determining process of human development, its values and priorities, key elements of life-style and behavior. In the context of reforming Russian higher school, the issue of factors playing their role in reproduction of health on youth seems to be actual from scientific and practical point of view. The key research problem of project is an issue of reproduction of students' health in institutional environment of university and how this environment effects health i.e. promotes development of habits of preservation and promotion of one's own health or develops unfavorable conditions for this process. The object of study were students entered the Federal university in 2015. The subject of study was reproduction of their health in university environment. Te article presents results of first stage of study of health of first course students from experimental group, elements of their life-style, self-preservation practices and subjective attitude to health. The initial data is presented to be analyzed hereinafter in dynamics in the process of further education of students in the university.

THE RESEARCH COMPETENCE OF PRACTICAL PHYSICIAN AS A FACTOR OF IMPROVEMENT OF PROFESSIONAL SELF-REALIZATION

Abstract

The article presents the analysis of impact of research competence on the process of professional self-realization of practical physicians. The personal self-perception of physician from positioning of oneself as active subject of cognition, opened to new experience and ready for getting out of available scope of knowledge is considered as an indicator of availability and developness of research competence at the stage of post-graduate stage of profession genesis. The article presents a complex of qualities of subject needed to successful self-realization in profession in conditions of modern medicine. The questionnaire survey was applied to assess through physicians the process of receiving and application of experience of research activities in university and also of the course of professional self-realization. The questionnaire emphasized such research skills as scientific informational, methodological, theoretical, empirical, written speech and communicative speech ones. The testing was applied to compare self-rating of physicians in the area of research skills and real research knowledge and skills. The results testify an inadequate attention to development of research competence at pre- and post-graduation work stages of professional genesis. The cohort of young specialists is considered as a most perspective one for working in this direction in state structures.

THE DEFICIENCY OF MEDICAL PERSONNEL: ARE THERE PERSPECTIVES FOR CHANGING?

Abstract

The attempts were made to analyze the causes of deficiency of the medical personnel in the Krasnoyarsk kraii and to study possible means of amelioration of situation in the region. The graduates of the Krasnoyarsk state medical university considered as a potential resource for elimination of medical personnel deficiency in regional health care system. The study of opinions of graduates of 2016 on specialty of «Medical business» on the basis of questionnaire survey. The development of questionnaire was implemented on the basis of pilot interviews. The data processing was implemented using licensed statistical software IBM SPSS Statistica 22.0. The methods of cluster analysis were applied alongside with calculations of descriptive statistics. The study was targeted to detecting factors effecting will of young specialists to work in districts of the Krasnoyarsk kraii and also establishing their ideas about further professional activities. All graduates were divided between 5 specific groups depending on their ideas about preferable conditions and place of employment. The respondents wishing to work in polyclinics are the only group where not salary nor conditions of work nor time of working under guidance of tutor are important and it's not easy to motivate them with anything else. More than one third of respondents perceive their own future without the limits of Krasnoyarsk. The key factors effecting wish to work in rural area are housing habitation, salary (higher than 5060 000 rubles), well-developed infrastructure and availability of competent physicians of narrow specialization at the given settlement.

THE SATISFACTION WITH MEDICAL SERVICES AS AN INDICATOR OF EFFICIENCY OF FUNCTIONING OF MEDICAL INSTITUTION

Abstract

The need in optimization of strategies of activities of various health care structures focuses scientific practical interest to studying a degree of satisfaction of patients with not only received medical care but psychological interactions with specialists, sanitary hygienic, organizational, sociocultural and other characteristics of curative institution. The purpose of study is targeted to corroborate a hypothesis about expediency of involvement of sociological surveys to medical care quality assessment in conditions of practical health care institutions. The article designates theoretical foundation of the problem and expounds results of applied study (questionnaire survey) targeted to evaluate competence and level of professionalism of specialists, benevolence of employees, diagnostic possibilities, sanitary hygienic conditions and also preventive, informational and organizational functioning of institution. On the basis of gender and age-related criteria differences in indices of requirements of respondents were established and their common needs in sphere of supporting physical and social health as well.
