THE SOCIAL INSTITUTION OF MEDICINE. PART I

Abstract

The concept of social institution correlates to period of becoming of sociology as a science and is related to classical works of O. Comte, E. Durkheim and G. Spencer. The institutional approach to studying society in classical interpretation includes five main social institutions ensuring stability of social system: family, state (politics), economics (manufacturing), education, religion. Nowadays, medicine is to be considered as an independent social institution and to proceed its investigation in the light of opening opportunities from positioning of sociology. The article minutely considers process of transformation of medicine into basic institution at the turn of XX-XXI centuries. At that it is emphasized that evolution process of institutionalization of medicine requires a new in-depth comprehension with regard to modern social transformational processes in the system of health protection. Simultaneously, an understanding is required concerning such fundamental aspects as changing situation in health care system, transformation of views of patient on process of medical care support and one's expectations from health care system, alterations of concepts of patient-physician relationships and re-arrangement of models of interaction of individual person with state, society, social institutions and groups.

THE SYSTEM OF MANDATORY MEDICAL INSURANCE ACCORDING APPRAISALS OF PATIENTS

Abstract

The awareness of population about essence of functioning of mandatory medical insurance system continues to be low. The purpose of study was analysis of opinions of residents of the Moscow oblast about this particular system. The sociological survey was carried out according standard technology using an original questionnaire. The public opinion was investigated among visitors of medical organizations in 2013 and 2017. According data of survey, the percentage of respondents with positive response to question concerning their awareness about the insurance company they are insured slightly increased in 2017. however, every thirteenth respondent responded negatively to this question. Only every sixth respondent addresses to the insurance company in case of occurrence of problems with medical care support. According data of both surveys, the right of choosing insurance company, medical organization and physician was applied only by 17% and 22% respondents correspondingly. In 2013, 20% of respondents addressed nowhere because of absence of problems in case of receiving medical care support. In 2017, a significant increase of addressing to administrations of medical organizations was established. Also, number of addressing to insurance medical organizations and foundations of mandatory medical insurance increased up to 13% and 27% correspondingly. The percentage of respondents that responded positively to question about unofficial compulsion to pay for medical services during four years decreased on 18% and made up to 12.6% out of 709 participants of survey. Thereby, from one hand activities related to informing population about occurring changes in health care system, including broadening of rights of citizen is not enough. From the other hand, citizen in number of cases have no interest in receiving necessary information and ignore it even including title of insurance medical company mentioned in mandatory medical insurance policy.

THE MEDICAL AWARENESS OF FUTURE MOTHERS ABOUT HEALTH OF NEWBORNS AND CHILDREN OF EARLY AGE

Abstract

The efficiency of preventive activities depends on medical awareness of population in issues of health. By force of this, the problem of increasing of awareness about issuers of health is considered by the WHO as the most actual one in area of public health. The particular interest is raised by awareness of female population - future mothers - concerning development of health of future children. The purpose of study is to analyze and evaluate awareness of pregnant women and involvement of obstetricians-gynecologists in development of health of newborns and children of early age. The sociological survey was carried out in the form of correspondence questionnaire survey of 350 pregnant women and 25 physicians. The study established low awareness of pregnant women about issues of development of health of children at the prenatal stage: 62.6% of women have no knowledge about effect of risk factors on health of future children, 59.3% experience emotional tension and 87% mention necessity of health education related to these issues. The vested interest of physicians in increasing of medical awareness of pregnant women is determined. It is established that most of obstetricians-gynecologists suffer from inadequate knowledge concerning issues of early diagnostic and primary prevention of inborn and inherited diseases. The obtained results determine necessity of implementation of organizational methodological activities targeted to increasing of medical awareness of women concerning their own health and understanding of personal responsibility for development of health of their future children.

Abstract

The problem of physician errors is resolved predominantly from position of empirical data. The related publications are inadequate due to subjectivism at disregarding methodological (formal logical and dialectic) laws of cognition of truth. This situation results in discrepancy and suspense of problem. most of the studies remain within the limits of critical but not positive (constructive) consideration of the problem. On balance authors receive no new knowledge. The article discusses methodological sources of subjective difficulties of implementing the role of physician. The solutions are proposed concerning certain aspects of the mentioned problem.

DOCTORS’ VIEWS ON THE ROLE OF PHARMACY IN MAINTAINING THE POPULATION HEALTH

Abstract

Pharmacy is the basic entity for the provision of pharmaceutical care. Its work is one of the factors that determine the population health in modern society. In order to identify the role of pharmacy in the system of public health protection a sociological study of the views of physicians (N=343), as an intermediate consumer of pharmaceutical care, was conducted. The study covered various aspects of the pharmacies’ work and identified the problems in the field of the population drug supply. Pharmacy is perceived by the medical community as a specialized organization that performs certain functions: dispensing of preferential medicines to separate categories of citizens, selling of medications with low prices and a wide range, production of medicines. Until now there is no integrated approach to the professional interaction between physician and pharmaceutical professional.

HEALTH AND WELL-BEING OF CITIZENS OF THE RUSSIAN FEDERATION IN EUROPEAN CONTEXT

Abstract

The article presents the analysis of the results of the comparative European Social Study related to health and psychological well-being obtained in the course of the third (2006) and Seventh (2014) waves. The indices of health self-appraisal, satisfaction with one's own life, health care system functioning, social security, feeling of happiness, personal attitudes to charity and environment protection. In majority of listed indices, the positive values of the citizen of Russia are on the last positions in the enumeration of 22 European countries. But while comparing data of 2014 and 2006 the responses of the citizen of Russia demonstrate increasing of positive values and decreasing of negative values. The detected positive trends permit to look for perspectives of social policy in health area with optimism. The reliability of obtained results is substantiated by data of studies of national sociological centers and statistical numbers concerning positive changes in sphere of life related to health of citizen of Russia.

Abstract

The diagnosis of HIV-infection conditions reconsidering by majority of HIV-infected individuals their identity and specificity of establishment of relationships in society considering new realities. The data related to social attitudes of mind of Russians to HIV-infected individuals obtained within the framework of original study, permitted to construct general concept about main characteristics attributed to representatives of this social group. The features of course of disease and its incurable character at the moment induce multitude of misgivings both in individuals with diagnosis of HIV-infection and healthy people that conditions wide-spread prevalence of negative and stigmatizing attitudes to HIV-infected people in modern Russian society. Nowadays, in Russians dominating attitude of mind to HIV-infected people still is intolerance against the background of inadequate awareness about problem of HIV-infection/AIDS.

Abstract

The oncologic diagnosis is a high-stress situation that determinatively effects the order of life of child, parents and relatives. The timely addressing for specialized medical care has a priority importance for favorable prognosis of course of disease. At that, the well-being of child with oncologic disease in many ways depends on social, economic, psycho-emotional resources that family disposes of. Besides, a significant potential of support of family and child in this situation have charity foundations, public organizations and volunteer associations (parental ones in the first place) which activities are targeted to supporting children with oncologic diseases.

THE COMPARATIVE ANALYSIS OF QUALITY OF LIFE OF ORPHAN CHILDREN IN CONDITION OF DISEASE

Abstract

The article presents comparative analysis of quality of life of orphan children in condition of disease. The study covered random sampling of 317 respondents aged from 14 to 18 years. The analysis of quality of life was carried out using questionnaire WHOQOL-100. The statistical reliability of difference of values of average indices of quality of life in groups of respondents was established by non-parametric criterion Mann-Whitney U-test. It is established that by all parameters the indices of quality of life of children residing in family (healthy ones and suffering of diabetes mellitus) are similar and alter insignificantly. At the same time, quality of life of orphan children is significantly lower, especially in group of ill children. The role of social factors impacting quality of life in condition of disease is determined. The established statistical dependence of indices of quality of life from disease and efficiency of re-socialization proves possibility of objectification of strategies of rehabilitation and development of system of personalized medical and social care of orphan children in condition of disease.
