THE SOCIAL INSTITUTION OF MEDICINE. PART II

Abstract

On the basis of the analysis of process of becoming (institutionalization) of social institute as a phenomenon the classical concept of five basic social institutes (family, education, religion, economic and political institutes) is broadened. The society in the course of its historical development forms new spectrum of needs hence the necessity in both reconsidering changes of functional tasks, limits, normative value and status role systems of social institutes in function and recognizing existence of new social institutes. At the modern stage, considering actual functional requirements of society, the social institute of medicine is a new social institute. The article considers process of transformation of medicine into independent social institute at the turn of XX-XXI centuries. At that it is noted that evolutionary process of institutionalization of medicine requires a new in-depth comprehension accounting actual social transforming processes in the health care system. Simultaneously, another comprehension is required related to altering situation in health care system itself, transformation of views of patient on process of rendering of medical care and expectations from health care system, alterations of concepts of patient-physician relationships, reorganization of models of interaction of a single individual with the state, society, social institutes and groups.

THE QUALITY OF LIFE OF PHYSICIANS IN THE CONTEST OF FINANCIAL DEPRIVATIONS

Abstract

The societal transformations and reforms of the Russian health care of last decades-initiated changes of professional status and stability of social professional groups of the institute of health care. The physicians occupied the lowest status grade level among the poorest stratum of population as a result of dragged out crisis and inefficient health care reforms. In modern Russia, alterations of social structure, increasing social differentiation, crisis and dysfunctional practices of Institute of health care resulted in marginalization of social professional groups that is discussed in the context of "concept of status non-consistence" developed by G. Lensky. The article presents the results of study concerning effect of financial deprivations to quality of life of physicians of the region. The evidences are presented related to socially determined and conditioned mainly by financial deprivations decreasing of quality of life of physicians working in medical organizations of various level in the Saratov oblast.

Abstract

The article considers main discourses of investigation of labor employment of graduates of medical universities as exposed in scientific publications. On the basis of analysis of relevant data array thematic sets of problems conditioning imbalance of labor market and inadequate efficiency of interaction of major actors involved into the mentioned process are marked out. The comprehension of nature of mechanisms of labor market accounting multi-value of the given occurrence acts as a necessary condition of development of effective strategy of solving the problem of employment of graduates of medical universities.

Abstract

The article presents the results of medical sociological study targeted to analyze concepts and motivational attitudes of students of medical universities related to healthy life-style and its components. The results demonstrate that students, though accepting importance of one's health promotion, committed to unhealthy life-style. They even more often refuse from regular exercises and are not enough active in issues of disease prevention and health promotion. The healthy life-style of students of medical universities is rather exclusion than prevalent practice.

THE STUDIES OF GENDER IDEOLOGY OF THE YOUTH: THE REVIEW OF FOREIGN PUBLICATIONS

Abstract

The article presents the results of foreign studies of gender ideology of students for a number of valuable social demographic variables. In the first part of publication the studies describing dynamics of gender ideology in various countries are analyzed. In the process of modernization of the Eastern Asia (China, Taiwan, South Korea, Japan), India and Indonesia female population is involved in work outside of home, a trend of egalitarianisation of gender relationship and spreading of families with two breadwinners. During transition from socialist to liberal states in the countries of the Eastern Europe the impact of religious conservative family’s values on the youth is less significant than that of Western ideas of individualization and permissiveness. In the developed countries (USA, Europe, Australia, Canada) gender revolution resulted in diversity of gender ideologies. At least in the European countries five models are fixed empirically: egalitarian, egalitarian essentialism, intensive parenthood, moderate conservative ideology. The second part of article presents the analysis of studies of attitudes of students in areas of gender and marriage and family relationships carried out in various countries that established that gender and religious identity are the major differentiating variables. The other characteristics such as urban rural origin, structure of parents' family, coeducation and separate education are less significant. The attitudes of the youth concerning social roles of males and females and future marriage are changing effected by peers, mass culture and personal experience. The conclusion is derived that in spite of more conservative attitudes of male youths factually in all countries, a slow convergence of views of male and female youths among well-educated strata. The denominational membership remains the main differential factor.

ABOUT DEVELOPMENT OF QUALITY OF ORGANIZATION AND PROVISION OF MEDICAL CARE

Abstract

Many issues concerning development of organization and and rendering of medical care continue to be unresolved in the health care of Russia. In spite of successes achieved efficiency of health care institutions functioning is established only in certain directions. The health care institutions are compelled more actively to get involved off-budget funding sources, to develop paid medical services and to reinvest received profits into material and technical resources. These occurrences rise before administrators of health care institutions new issues related first of all to efficiency of management, establishment of functional organizational structures, achievement of profitability and development stability. The solving of many key issues depends on administrators of health care institutions considering issues of quality. Therefore, the study was carried out concerning the attitude of health care professionals to the issue of developing quality of health care institutions through amelioration of quality of organization and management.

THE MODERN APPROACHES TO EVALUATION OF QUALITY OF LIFE OF PATIENTS WITH CRANIOCEREBRAL TRAUMA

Abstract

The actuality of problem of quality of life of patients with craniocerebral trauma is involved with high prevalence of the mentioned pathology and uncertainty of medical social prognosis. The outcomes of craniocerebral trauma, even in case of its light degree, for the first time can be manifested after many years being besides not always adequate to severity of course of acute period of craniocerebral trauma and having deteriorative course. The article presents the results of original study of quality of life of patients of able-bodied age with old craniocerebral trauma. The degree of severity of old craniocerebral trauma is considered as an obligate predictor of quality of life of patients.
