Аннотация
Хронические заболевания, бремя которых рассматривается как одна из ведущих проблем национальных систем здравоохранения, существенно влияют на качество жизни и способствуют формированию витальных ограничений в повседневной жизни. Болезнь Крона и язвенный колит — наиболее распространённые формы воспалительных заболеваний кишечника. Пациенты с воспалительным заболеванием кишечника вынуждены жить с длительным и тяжёлым заболеванием; при этом некоторые симптомы сохраняются в период клинической ремиссии и негативно сказываются на социальных отношениях, личной, профессиональной и бытовой сферах жизни. Цель — рассмотреть витальные ограничения пациентов с воспалительными заболеваниями кишечника по данным литературы. Проведён систематический обзор по методологии PRISMA (Preferred reporting items for systematic reviews and meta-analyses). При отборе релевантных публикаций использованы базы данных PubMed и eLibrary.Ru за период 2001–2024 гг. В итоговый анализ включены 54 публикации, отобранные по ключевым словам: «соматические и социальные ограничения при воспалительных заболеваниях кишечника», «качество жизни», «бремя хронических заболеваний», «somatic and social limitations in inflammatory bowel diseases», «quality of life», «the burden of chronic diseases», а также по спискам литературы найденных источников. Критерии включения: оригинальные исследования, обзоры литературы и метаанализы, содержащие данные о витальных ограничениях при воспалительных заболеваниях кишечника. Исключены клинические случаи, авторские мнения. Витальные ограничения пациентов с воспалительными заболеваниями кишечника рассмотрены с позиций социологии повседневности, в рамках которой они трактуются как барьеры в повседневной жизни, а также социологии медицины, раскрывающей социальное восприятие здоровья, жизни с хроническим заболеванием и общественное отношение к пациенту. В научных исследованиях рассматриваются витальные ограничения, возникающие у пациентов с воспалительными заболеваниями кишечника или осложнениями лечения, охватывающие различные аспекты жизнедеятельности. Принимая во внимание их влияние на качество жизни и клинический прогноз, данные ограничения следует учитывать при формировании континуума интегрированной медико-социальной помощи пациентам с воспалительными заболеваниями кишечника.
