The relevance of this study lies in the fact that the disease is associated with prolonged diagnostic delay, a marked decline in quality of life, economic losses, and disruptions to reproductive plans, while its social and demographic effects remain insufficiently conceptualized from an interdisciplinary perspective. The scientific novelty of the study lies in examining endometriosis as a factor in the reproduction of social inequality, intensifying disparities in income, place of residence, and access to medical care. The article presents the results of a search and analytical study based on a review of international and Russian publications, materials from international organizations, clinical guidelines, and studies devoted to patients’ diagnostic pathways, quality of life, economic burden, work-related losses, and the impact of endometriosis on fertility. It is shown that diagnostic delay across countries is measured in years and is caused by a combination of behavioral, professional, and institutional barriers. The social consequences of the disease are systematized, including chronic pain, psycho-emotional burden, reduced work productivity, rising direct and indirect costs, as well as changes in family and reproductive strategies. Attention is paid to the demographic effects of endometriosis through infertility, delayed motherhood, and the growing demand for assisted reproductive technologies. In the Russian context, institutional constraints related to the uneven availability of specialists, diagnostic services, and long-term hormonal therapy are identified. It is concluded that endometriosis serves as a significant channel for the accumulation and reproduction of social inequality. The article makes it possible to identify the mechanisms through which a medical problem is transformed into a socio-economic and demographic risk and substantiates the need for comprehensive measures in healthcare and social policy.</p></abstract><trans-abstract xml:lang="ru"><p>Эндометриоз относится к числу наиболее распространённых хронических заболеваний женского репродуктивного здоровья, но его значение выходит далеко за пределы клинической проблемы. Актуальность работы обусловлена тем, что заболевание сопровождается длительной диагностической задержкой, выраженным снижением качества жизни, экономическими потерями и нарушением репродуктивных планов, при этом его социальные и демографические эффекты остаются недостаточно осмысленными в междисциплинарной перспективе. Научная новизна исследования заключается в рассмотрении эндометриоза как фактора воспроизводства социального неравенства, усиливающего различия по доходу, месту проживания и доступу к медицинской помощи. В статье представлены результаты поисково-аналитической работы, основанной на обзоре международных и отечественных публикаций, материалов международных организаций, клинических рекомендаций и исследований, посвящённых диагностическому пути пациенток, качеству жизни, экономическому бремени, трудовым потерям и влиянию эндометриоза на фертильность. Показано, что задержка диагностики в разных странах измеряется годами и обусловлена сочетанием поведенческих, профессиональных и институциональных барьеров. Систематизированы социальные последствия заболевания, включая хроническую боль, психоэмоциональную нагрузку, снижение трудовой продуктивности, рост прямых и косвенных расходов, а также изменение семейных и репродуктивных стратегий. Особое внимание уделено демографическим эффектам эндометриоза через бесплодие, отсрочку материнства и рост спроса на вспомогательные репродуктивные технологии. В российском контексте выявлены институциональные ограничения, связанные с неравномерной доступностью специалистов, диагностики и длительной гормональной терапии. Сделан вывод о том, что эндометриоз выступает значимым каналом накопления и воспроизводства социального неравенства.
