Newborns with extremely low birth weight are at high risk of developing disabling conditions and experiencing reduced overall quality of life.</p> <p><bold>AIM:<italic> </italic></bold>The work aimed to analyze pediatricians’ opinions on issues related to the birth of extremely low birth weight infants.</p> <p><bold>METHODS:<italic> </italic></bold>A cross-sectional survey was conducted in 2023–2024 in medical organizations of Saratov using an author-designed questionnaire. A total of 106 pediatricians with over 5 years of professional experience participated.</p> <p><bold>RESULTS:<italic> </italic></bold>The study addressed decision-making regarding the need for resuscitation of extremely low birth weight infants and the permissibility of withdrawing life-support systems in cases of severe disabling conditions.</p> <p>The majority of pediatricians (64.7%) disagreed with the current birth criteria. A total of 32.7% of respondents opposed resuscitative efforts for such newborns. Most participants (62.3%) reported never facing unfounded accusations of negligent homicide of newborns, which they attributed to the specifics of their work. Nearly half (46.7%) of respondents believed that the survival of extremely low birth weight infants results in a sharp deterioration of pediatric population health indicators.</p> <p><bold>CONCLUSION:<italic> </italic></bold>Most pediatricians disagree with the current birth criteria. Their opinions are shaped by the uniquely high level of professional responsibility, the complexity of decision-making, unpredictable outcomes, and experiences of unjust accusations in the death of extremely low birth weight infants. The respondents emphasized the necessity for both parents to participate in decision-making regarding resuscitation, palliative care, and withdrawal of life-support systems in the event of disabling conditions in extremely low birth weight infants.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование. </bold>В 2011 году в России приняты новые стандарты медицинских критериев рождения, соответствующие рекомендациям Всемирной организации здравоохранения от 1975 года. Новорождённые с экстремально низкой массой тела входят в группу высокого риска по формированию инвалидизирующей патологии и снижению общего качества жизни.</p> <p><bold>Цель исследования. </bold>Проанализировать мнение врачей-педиатров по вопросам, связанным с рождением детей с экстремально низкой массой тела.</p> <p><bold>Методы. </bold>Одномоментное опросное исследование проведено в 2023–2024 гг. в медицинских организациях г. Саратова с использованием авторской анкеты. В нём приняли участие 106 врачей-педиатров со стажем работы более 5 лет.</p> <p><bold>Результаты. </bold>В исследовании обсуждались проблемы принятия решений о необходимости реанимации новорождённых с экстремально низкой массой тела, а также о допустимости отключения их от систем жизнеобеспечения при развитии инвалидизирующих заболеваний. Большинство (64,7%) врачей-педиатров не согласны с действующими критериями рождения. Против проведения реанимационных мероприятий таким новорождённым высказались 32,7% респондентов. Не сталкивались с необоснованными обвинениями в причинении смерти новорождённым по неосторожности 62,3% участников опроса (что связано со спецификой работы). Почти половина (46,7%) респондентов полагают, что выхаживание новорождённых с экстремально низкой массой тела приводит к резкому ухудшению показателей здоровья детского населения.</p> <p><bold>Заключение. </bold>Большинство врачей-педиатров не согласны с текущими критериями рождения. На их мнение повлияли уникально высокий уровень профессиональной ответственности, сложность принятия решений и непредсказуемость последствий, а также случаи несправедливых обвинений в смерти новорождённых с экстремально низкой массой тела.
