The patients’ age ranged from 23 to 78 with an average age of (47±1.7), including 48 (55.8%) men and 38 (44.2%) women. In 47 patients (54.6%), cerebral aneurysm was confirmed by selective cerebral angiography. The aneurysm size ranged from miliary (3 mm) to giant (25–30 mm).</p> <p><bold>RESULTS:</bold><italic> </italic>In patients with ruptured cerebral aneurysms (CAs), clinical findings were associated with extensive subarachnoid hemorrhage, site and volume of intracerebral hematomas. In patients with unruptured aneurysms, the findings were associated with the site and the size of CAs. Rupture of the aneurysm sac at different stages of the surgery (15.1% of cases) is a severe intraoperative complication. The worst outcomes with a mortality rate of up to 4.17% were observed in patients with ruptured CAs who have Hunt &amp; Hess grade III and IV, and Glasgow Coma Scale scores of 7–8 and 9–10. If it is difficult to directly access the aneurysm, it is reasonable to perform microsurgical subpial resection of abnormal brain tissues around the aneurysm to avoid traumatic injury of aneurysm walls, and artery spasm. To avoid aneurysmal rupture when isolating the aneurysmal neck, it is reasonable to temporarily use special removable (temporary) clips on the proximal and distal segments of the carrier vessel. Once the aneurysmal neck has been isolated, a special clip is applied. If possible, the patency and integrity of the carrier vessel and its branches shall be preserved. To confirm that there is no blood flow in the aneurysm and assess the arterial patency, we recommend to use intraoperative microvascular Doppler ultrasonography.</p> <p><bold>CONCLUSION:</bold><italic> </italic>The outcome of surgical treatment for ruptured aneurysms depended on the preoperative patient status. Microsurgical cerebral aneurysm (CA) clipping effectively prevents recurrent subarachnoid hemorrhage. While the patients may have no intraoperative and early postoperative complications, late complications requiring additional surgery are still possible.</p></abstract><trans-abstract xml:lang="ru"><p><bold>Обоснование.</bold> Несмотря на усовершенствование методов диагностики и микрохирургической техники лечения артериальных аневризм (АА) головного мозга, сохраняется высокая вероятность летальности и стойкой инвалидности среди трудоспособного населения.</p> <p><bold>Цель.</bold> Определить факторы, влияющие на эффективность оперативных вмешательств у пациентов с АА головного мозга.</p> <p><bold>Методы.</bold> В работе представлены результаты обследования и хирургического лечения 86 пациентов, оперированных в период с 2017 по 2023 год в клинике нейрохирургии Национального госпиталя Кыргызской Республики. Возраст пациентов составил от 23 до 78 лет, средний возраст — (47±1,7) лет. Среди них мужчин было 48 (55,8%), женщин — 38 (44,2%). У 47 пациентов (54,6%) аневризма сосудов головного мозга подтверждена селективной церебральной ангиографией. Размеры аневризм колебались от милиарных (3 мм) до гигантских (25–30 мм).</p> <p><bold>Результаты.</bold> Клиническая картина у пациентов с разорвавшимися АА была связана с распространённостью субарахноидального кровоизлияния, локализацией и объёмом внутримозговых гематом, у пациентов с неразорвавшимися аневризмами — с расположением и размерами АА. Одно из серьёзных интраоперационных осложнений — разрыв аневризматического мешка на разных этапах операции (15,1% случаев). Наихудшие результаты с летальностью до 4,17% отмечались у пациентов с разоравшимися АА и степенью тяжести состояния III и IV по шкале Ханта–Хесса, 7–8 и 9–10 баллов по шкале Глазго. В тех случаях, когда прямой доступ к аневризме затруднён, целесообразно произвести субпиальную микрорезекцию изменённого мозгового вещества вокруг аневризмы. Это позволяет избегать травматического повреждения стенок аневризмы и развития спазма артериальных сосудов. Чтобы избежать разрыва аневризмы при выделении шейки аневризмы, целесообразно временно накладывать специальные съёмные (временные) клипсы на проксимальный и дистальный сегменты несущего сосуда аневризмы. После того как удалось выделить шейку аневризмы накладывали специальный клипс и по возможности сохраняли проходимость и целостность несущего аневризму сосуда и его ветвей. Для определения отсутствия кровотока в аневризме и оценки проходимости артерий рекомендуется применять интраоперационную микрососудистую допплерографию.</p> <p><bold>Заключение.</bold> Результаты хирургического лечения при разорвавшихся аневризмах зависели от предоперационного состояния пациентов. Микрохирургическое клипирование АА эффективно предотвращает повторное субарахноидальное кровоизлияние.
